About the role
This role reports to the Manager of Support Services and is based in the Columbus Service Area, with a schedule of Monday-Friday, 8:00 am to 4:30 pm, including on-call and holiday rotation.
It involves providing counseling and supportive services to help patients and families cope with terminal illness, including psychosocial assessments, grief counseling, and assistance with legal and financial matters.
Qualifications include a Master’s in Social Work, Indiana licensure (LSW or LCSW), and relevant experience, preferably in hospice or home health care. Strong interpersonal, organizational, and communication skills are essential. The position requires physical ability to perform job duties and adherence to safety protocols.

- Develops and revises care plans, coordinates hospice services, and participates in team meetings.
- Facilitates support groups and memorial services, and assists with bereavement follow-up.
- Provides education to professionals and volunteers, and participates in quality improvement activities.
