About the role
This position reports to the Manager of Support Services and is based in the Columbus Service Area, working Monday to Friday, 8:00 am to 4:30 pm, with on-call and holiday rotation.
Job Summary: Responsible for providing counseling and supportive services to patients and families coping with terminal illness, fostering adjustment and grief processing.
Responsibilities Include
Qualifications: Master’s in Social Work (MSW), Indiana licensure (LSW or LCSW), and at least one year of healthcare social work experience preferred. Requires excellent interpersonal, organizational, and communication skills, proficiency in Microsoft Office, and a valid Indiana driver’s license.
Work Conditions: Mainly indoor environment, occasional lifting, and exposure to blood-borne pathogens, with physical demands including standing, walking, and reaching. Flexibility to work evenings and weekends is necessary.

- Developing and implementing care plans, offering psychosocial assessments, and providing counseling for emotional, social, and financial concerns.
- Assisting with legal document location, coordinating hospice services, facilitating support groups, and participating in team meetings and quality improvement efforts.
- Supporting bereavement follow-up, facilitating memorial services, and training volunteers.
