About the role
This exempt position reports to the Support Services Manager and is based in the Columbus Service Area, with a schedule of M-F 8-4:30 pm, including on-call and holiday rotations.
Job Summary: Responsible for providing counseling and supportive services to facilitate grief and patient/family adjustment to terminal illness, tailored to individual needs.
Working Conditions: Mainly indoor with some outdoor exposure, requiring occasional lifting, repetitive motion, and adherence to safety protocols. Must have good vision, dexterity, and communication skills.
Qualifications: MSW degree from an accredited program, Indiana LSW or LCSW license, valid Indiana driver’s license, and hospice or home health experience preferred. Must demonstrate strong interpersonal, organizational, and communication skills, proficiency in Microsoft Office, and ability to handle high-stress situations. Flexibility to work evenings and weekends is required.

- Develops and revises care plans, providing services aligned with these plans.
- Assists patients/families with legal documents like Living Wills and POA.
- Performs psychosocial assessments and offers counseling on adjustment, stress, anxiety, depression, and grief.
- Coordinates hospice services, environmental needs, and community support.
- Provides economic counseling for financial aid applications.
- Documents social findings and plans in patient charts.
- Participates in quality improvement, team meetings, and facilitates support groups and memorial services.
- Assists with bereavement follow-up and trains volunteers.
- Participates in on-call services to ensure 24/7 availability.
- Engages in educational activities and performs other duties as assigned.
