About the role
This exempt position reports to the Support Services Manager and serves the Columbus Service Area, including multiple communities.
It involves providing counseling and supportive services to patients and families coping with terminal illness, focusing on grief, adjustment, and psychosocial needs.
Responsibilities include developing and implementing care plans, offering psychosocial and psychological assessments, assisting with legal and financial forms, coordinating hospice services, facilitating support groups, and providing grief and bereavement follow-up.
The role requires participation in team meetings, quality improvement, and volunteer training, with on-call availability.
Physical demands include light lifting and environmental exposure.
Qualifications include a Master of Social Work (MSW), Indiana licensure (LSW or LCSW), valid driver’s license, and preferred hospice experience.
Strong interpersonal, organizational, and communication skills are essential, along with proficiency in Microsoft Office.
