About the role
This position reports to the Manager of Support Services and is based in the Columbus Service Area, working M-F 8-4:30 with on-call and holiday rotation.
It involves providing counseling and supportive services to patients and families dealing with terminal illness, including psychosocial assessments, grief counseling, and assistance with legal and financial matters.
Responsibilities include developing and implementing care plans, coordinating hospice services, facilitating support groups, participating in team meetings, and assisting with bereavement follow-up.
The role requires a Master’s in Social Work (MSW), Indiana licensure (LSW or LCSW), and at least one year of healthcare social work experience.
Qualifications include strong interpersonal, organizational, and communication skills, proficiency in Microsoft Office, and the ability to work under stress.
Physical demands involve occasional lifting, and the work environment includes inside and outside conditions. Reasonable accommodations are available for qualified individuals.
