- Consistency, clinical autonomy, and team support
- In-person care at local Post-Acute & Skilled Nursing Facilities
- Collaborative care with physicians and on-site teams
Compensation & Benefits
- Salary: $100,000–$120,000 annually (based on experience)
- Performance bonus, $1,500 CME stipend, paid holidays
- Health, Dental, Vision, 401(k), PTO
Qualifications
- Active SC Nurse Practitioner license (or ability to obtain)
- Board Certified (ANCC, AANP, or equivalent)
- Experience in post-acute, hospital, or outpatient care preferred
- Passion for geriatric, preventative, and chronic care
