Description STATUS: Full-time, 37.5 hours a week, Non-exempt HOURLY RATE: up to $25.00 per hour Summary Lifespan is a trusted, mission-driven, regional nonprofit dedicated to helping older adults take on the challenges and opportunities of longer life. The Program Coordinator triages and assigns new referrals to the Lifespan Community Care Connections (CCC) team. This position communicates with internal and external referral sources to gather necessary information and resources, so clients are served as timely as possible. Care options, solutions and guidance navigating challenging systems is provided along with linkage to appropriate services. The Program Coordinator maintains relationships with professional referral sources and supports the program director by providing education about Lifespan care management and the CCC program. Duties And Responsibilities
Review and process CCC program referrals
Review all referrals that come through the CCC referral line or email
Triage and assign referrals to the team
Communicate with referral sources
Update client system database
Work collaboratively with the LPN Program Coordinator
Provide Care Management
Accept case assignments and assist team in reducing delays in service
Complete comprehensive in-home assessments to determine needs
Create and implement care plans that meet client needs and preferences
Monitor status of care plans and revises care plans as needed
Provide information and referral, connection to long-term care services, advocacy and benefit & entitlement counseling
Assist with Quality Assurance activities
Participate in program chart reviews, as directed
Communicate with Director any quality assurance concerns
Participate as a member of the inter-agency team
Participate in all Lifespan staff meetings and trainings
Participate in all CCC program meetings and trainings
Participate in Quality Assurance activities
Accept and seek guidance and supervision
Timely completion of all required documentation and data entry into appropriate database.
Maintain up to date professional knowledge of the geriatric field, with special focus on community services, entitlement benefits, clinical skills and competencies through reading, workshops, and conferences.
Is aware of and actively supports Lifespan’s initiative of being collaborative and community- focused, innovative and culturally competent.
Other duties as assigned by supervisor.
Requirements QUALIFICATIONS EDUCATION: Bachelor or Master of Social Work or equivalent degree in the fields of health and human services. EXPERIENCE: At least one year of experience coordinating community-based services for older adults. Physical And Mental Requirements
Must be able to meet with clients in their homes.
A portion of the work is performed at a desk or at a personal computer station on Lifespan premises.
Excellent verbal, written and telephone communication skills are required.
Strong written and computer skills needed.
Interface with all levels of personnel.
Ability to maintain composure under strict deadlines and difficult situations.
Ability to work in multi-cultural situations.
Ability to self-manage time and schedule.
Access to reliable transportation.
Valid NYS Driver’s license
