What To Expect (Job Responsibilities)
Prepare members for telehealth visits, ensuring they feel comfortable and at ease
Coordinate care by working with home health partners and managing durable medical equipment (DME) needs
Notify members of lab results and assist in coordinating follow-up care as directed by clinicians
Advocate for members by addressing care coordination issues through effective communication
Review internal program referrals and send educational materials to members
What Is Required (Qualifications)
MA Certification (required), including expired certifications
Fully bilingual in English and Spanish
Minimum of two (2) years of patient-facing experience, with a preference for telehealth experience
Prior primary care experience is a plus
Strong communication skills to effectively interact with various stakeholders
How To Stand Out (Preferred Qualifications)
Experience in care coordination, particularly with Medicare patients
Highly organized and able to manage a large panel of members
Comfortable using technology, including electronic health records and various computer platforms
Ability to thrive in a fast-paced and rapidly growing environment
Passionate about helping others navigate the healthcare system with empathy and warmth
