What To Expect (Job Responsibilities)
- Prepare members for telehealth visits, ensuring they feel comfortable and at ease
- Coordinate care by working with home health partners and managing durable medical equipment (DME) needs
- Notify members of lab results and assist in coordinating follow-up care as directed by clinicians
- Advocate for members by addressing care coordination issues through effective communication
- Review internal program referrals and send educational materials to members
What Is Required (Qualifications)
- MA Certification (required), including expired certifications
- Fully bilingual in English and Spanish
- Minimum of two (2) years of patient-facing experience, with a preference for telehealth experience
- Prior primary care experience is a plus
- Strong communication skills to effectively interact with various stakeholders
How To Stand Out (Preferred Qualifications)
- Experience in care coordination, particularly with Medicare patients
- Highly organized and able to manage a large panel of members
- Comfortable using technology, including electronic health records and various computer platforms
- Ability to thrive in a fast-paced and rapidly growing environment
- Passionate about helping others navigate the healthcare system with empathy and warmth
