Job Title: Clinical Pharmacist Location: 302 S Grand Ave, Sun Prairie, WI 53590 Employment Type: Full Time Schedule: Monday – Friday 8am – 7 pm, Saturday 9am – 1pm, On-call rotation Job Summary Pharmacists are responsible for the daily operation of the pharmacy and encompasses the job description of every job below them in the pharmacy structure. Additionally, pharmacists supervise all technicians and support staff and assuring medications are delivered to the patient in a safe, accurate and timely fashion. The Clinical Pharmacist in a community setting is responsible for delivering patient-centered clinical services while supporting safe, effective, and cost-conscious medication use. This role integrates traditional dispensing functions with expanded clinical responsibilities such as medication therapy management (MTM), chronic disease state management, patient education, and collaboration with healthcare providers. Key Responsibilities
Accurately review, verify, and dispense prescription medications
Provide patient counseling on medication usage, side effects, and interactions
Maintain complete and accurate patient profiles in pharmacy systems
Review prescriptions for clinical appropriateness and safety
Oversee pharmacy technicians and support staff, including workflow delegation and accuracy checks
Resolve insurance processing issues and communicate with medical professionals
Monitor inventory, order medications, manage controlled substances, and ensure proper medication storage and handling
Maintain compliance with state/federal pharmacy laws and company policies
Ensure patient confidentiality and HIPAA compliance
Provide comprehensive Medication Therapy Management (MTM) services, including medication reviews and care plan development
Manage chronic disease states (e.g., diabetes, hypertension, hyperlipidemia, asthma) under collaborative practice agreements (if applicable)
Monitor patient outcomes and adjust therapy in collaboration with prescribers
