The Pharmacy Technician is responsible for providing member medication counseling and assistance to minimize gaps in treatment by collaborating with various teams across the Health Plan and with community pharmacists and providers. Duties & Responsibilities:
Performs review of medication lists to understand and eliminate barriers to medication therapy while serving as the member’s Pharmacy Navigator to support medication adherence, minimize gaps in therapy, and prevent medication-related adverse events
Facilitates member access to medications by obtaining refills, coordinating new prescriptions, scheduling appointments, assisting members experiencing financial barriers, aligning refills to reduce pharmacy visits, documenting care delivered, and communicating updates to providers
Partners with Pharmacists and other internal and external partners to communicate medication treatment plans and coordinate MTM interventions within broader healthcare management services
Collaborates with community pharmacies to monitor and communicate member medication adherence
Conducts patient health assessments to collect medication profile information including prescriptions, OTC medications, and herbal therapies to identify barriers impacting adherence or access to care
Handles member and provider communications telephonically
Performs additional duties as assigned
Minimum Qualifications:
Strong written and verbal communication skills with the ability to communicate professionally and clearly
Experience communicating with patients, providers, and pharmacists regarding medication-related concerns
Active State Pharmacy Technician registration/license or national certification (PTCB or NHA) in good standing
High School Diploma or GED
Preferred Qualifications:
Bilingual skills
Medication Therapy Management (MTM) Certification
Experience partnering with community pharmacies to support medication adherence and compliance initiatives
