Job Summary:
We are seeking a dedicated and skilled Physical Therapist to join our home health care team on a. As a Home Health Physical Therapist, you will play a crucial role in providing high-quality rehabilitation services to patients in the comfort of their homes. The ideal candidate is compassionate, knowledgeable, and committed to helping individuals regain their independence and improve their overall well-being.
Responsibilities:
- Conduct comprehensive assessments of patients' physical conditions.
- Develop and implement individualized treatment plans based on assessment findings.
- Provide hands-on therapy and exercises to promote mobility, strength, and function.
- Educate patients and their families on therapeutic exercises and activities.
- Collaborate with other healthcare professionals, including nurses, physicians, and social workers, to ensure coordinated care.
- Document patient progress, treatment plans, and outcomes accurately and timely.
- Monitor and adjust treatment plans as needed to achieve optimal patient outcomes.
- Maintain compliance with all relevant regulations and standards.
Qualifications:
- Valid state AZ licensure as a Physical Therapist.
- Will accept Physical Therapists without home health experience
- Strong clinical and interpersonal skills.
- Excellent communication and documentation skills.
- Ability to work independently and as part of a collaborative healthcare team.
- Knowledge of and adherence to professional, ethical, and legal standards.
