Community Relations HHS Hunterdon Healthcare System Inc., 2100 Wescott Drive, Flemington, NJ Full-time M-F 8am-4:30pm with occassional nights/weekend days, Day Shift w/ occasional Evening or Night Hiring Range: $19.00 - $23.75 Hourly Req # 79511 Position Summary Under the supervision of the Community Outreach Coordinator, and the direction of the Director of Community Relations, the Community Health Worker (CHW) provides essential support for hospital and grant-funded health initiatives, focusing on improving healthcare access and outcomes for everyone in the Hunterdon Health service area with a particular focus on uninsured, and underinsured populations. The CHW serves as a bridge between the clinical setting and the community, engaging patients directly to address social determinants of health and ensure connection to preventive services, screenings, medications, and community resources. Qualifications Knowledge, Skills, and Abilities Required
Strong written and verbal communication skills, including the ability to perform public speaking for small and large groups.
Proficient computer skills: Google Workspace, Microsoft Office Suite (Excel, Word, PowerPoint), data management software.
Strong organizational skills and the ability to work both independently and as a productive member of a team.
Valid driver’s license and reliable transportation.
Preferred
Bilingual (Spanish/English) is highly preferred to effectively serve diverse community populations.
Local knowledge of community resources in the Hunterdon Health service area is preferred.
Experience working within an Electronic Medical Record (EMR)
