SIGN ON BONUS: Includes a Sign on Bonus up to $5,000 Position Summary :The Occupational Therapist (OT) provides occupational therapy services to patients in accordance with the established plan of care and consistent with the philosophy, objectives, and policies of the agency. Qualifications :
Licensed as an Occupational Therapist by the State of Kansas.
Has graduated from an OT curriculum approved by the American Occupational Therapy Association or the Council of Medical Education and Hospitals of the AMA.
Minimum of 1 years of experience as an Occupational Therapist in a clinical care setting preferred.
Prior home healthcare experience preferred.
Valid state driver's license and reliable transportation required.
Maintains annual TB test, CPR certification, and other required documents.
Responsibilities :
Assist physicians in evaluating patients’ functional levels and developing treatment plans and goals
Perform initial evaluations within 48 hours of referral and re-evaluations every 30 days, as appropriate
Coordinate plans of care with physicians and other healthcare professionals as needed
Prepare accurate clinical documentation, including evaluations, progress notes, and summaries
Revise treatment plans and notify the physician of any changes in patient status or care needs
Educate patients and families on therapy programs, equipment use, and safe patient management
Observe, document, and report changes in patient condition or schedule to agency leadership
Collaborate with nursing staff and other disciplines to ensure coordinated patient care
Provide staff in-services as requested
Participate in chart reviews, case conferences, peer review, and QAPI activities
Communicate effectively with patients, caregivers, families, and agency staff
Ensure all duties are completed accurately and in a timely manner
Perform other duties as assigned
