Position Summary :The Occupational Therapist (OT) provides occupational therapy services to patients in accordance with the established plan of care and consistent with the philosophy, objectives, and policies of the agency.
Qualifications :
- Licensed as an Occupational Therapist by the State of Kansas.
- Has graduated from an OT curriculum approved by the American Occupational Therapy Association or the Council of Medical Education and Hospitals of the AMA.
- Minimum of 1 years of experience as an Occupational Therapist in a clinical care setting preferred.
- Prior home healthcare experience preferred.
- Valid state driver's license and reliable transportation required.
- Maintains annual TB test, CPR certification, and other required documents.
Responsibilities :
- Assist physicians in evaluating patients’ functional levels and developing treatment plans and goals
- Perform initial evaluations within 48 hours of referral and re-evaluations every 30 days, as appropriate
- Coordinate plans of care with physicians and other healthcare professionals as needed
- Prepare accurate clinical documentation, including evaluations, progress notes, and summaries
- Revise treatment plans and notify the physician of any changes in patient status or care needs
- Educate patients and families on therapy programs, equipment use, and safe patient management
- Observe, document, and report changes in patient condition or schedule to agency leadership
- Collaborate with nursing staff and other disciplines to ensure coordinated patient care
- Provide staff in-services as requested
- Participate in chart reviews, case conferences, peer review, and QAPI activities
- Communicate effectively with patients, caregivers, families, and agency staff
- Ensure all duties are completed accurately and in a timely manner
- Perform other duties as assigned
