Position Details
- Contract per visit — flexible schedule

Responsibilities
- Conduct skilled occupational therapy evaluations and treatment visits in the patient's home
- Complete and submit OASIS assessments accurately and on time
- Develop and implement individualized plan of care in coordination with the physician and care team
- Provide ADL and IADL retraining including bathing, dressing, grooming, meal preparation, and home management
- Address fine motor skills, upper extremity strengthening, and coordination
- Recommend and train patients and caregivers on adaptive equipment and home modifications
- Conduct home safety evaluations and fall prevention strategies
- Educate patients and caregivers on energy conservation and compensatory techniques
- Maintain timely documentation in compliance with Medicare and agency standards
- Communicate clinical updates and changes in patient condition to the care team
Requirements
- Active California Occupational Therapy license in good standing
- Minimum 1 year of experience required
- OASIS competency required — must be able to complete independently
- CPR certification current
- Reliable transportation and valid California driver's license
- Strong documentation and communication skills
