Role Description This is a part-time, hybrid role for a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Counselor (LPC) located in Vernon, CT, with some flexibility for remote work. The professional will provide direct mental health support to oncology patients, survivors, caregivers, and families. Responsibilities include conducting psychosocial assessments, delivering therapeutic interventions, collaborating with interdisciplinary teams, offering case management services, and developing personalized care plans aligned with patients' emotional and mental health needs. Qualifications
Expertise in Social Work and Medical Social Work, with licensure as LCSW, LMFT, or LPC in the state of Connecticut
Experience in Case Management and ability to coordinate and advocate for client needs
Strong understanding of Mental Health and Behavioral Health care, especially in the context of oncology
Empathy, active listening, and excellent communication skills to support individuals and families during challenging times
Proficiency in working within an interdisciplinary team and developing holistic care plans
Master’s degree in social work, counseling, or a related field
Experience in oncology, palliative care, or chronic illness care is highly desirable
Ability to work independently while adhering to professional and ethical standards
