**The Opportunity **

We’re looking for an ambitious licensed Physical Therapist to join Bloom360 as our Lead PT. This is a telehealth role where you’ll lead our movement program, working directly with members on overall fitness, injury prevention, chronic pain, and rehabilitation. You'll collaborate closely with our physician and dietitian and help shape how movement counseling gets delivered at Bloom360.

**What You'll Do **

- Conduct virtual functional movement assessments to evaluate mobility, strength, pain, and functional limitations within the context of each patient’s medical history

- Design and progress individualized home exercise programs (HEPs) that are evidence-based and aligned with patient goals and clinical conditions

- Deliver telehealth physical therapy across full scope, including therapeutic exercise, functional training, and pain management

- Collaborate closely with physicians and care team to integrate movement into overall care

- Support patients with musculoskeletal conditions, chronic pain, post-surgical recovery, and mobility limitations using modern pain science and functional rehabilitation principles

- Apply behavior change strategies to help patients build sustainable movement habits and overcome barriers (e.g., pain avoidance, time constraints, attitude)

- Educate patients on injury prevention, ergonomics, and safe, effective movement in daily life

- Track progress using functional and patient-reported outcomes, adjusting care plans accordingly

- Contribute to development of Bloom360’s movement care model, resources, and protocols

- Maintain accurate, timely documentation within the platform

**What We're Looking For **

Required Qualifications:

- Doctor of Physical Therapy (DPT) or licensed Physical Therapist (PT)

- Tech savvy with strong preference for demonstrated telehealth PT experience

- Strong understanding of functional movement screening, exercise prescription, and progressive programming

- Clear, confident communicator who can coach and educate members effectively over video

- Michigan licensure (DPT/PT)

Preferred Qualifications:

- Experience treating chronic pain, musculoskeletal conditions, or post-surgical rehab via telehealth

- Familiarity with prescribing exercise as medicine for metabolic and cardiovascular conditions

- Experience building video-based exercise libraries or member home program resources

**The Opportunity **

We are seeking a passionate Registered Dietitian (RD or RDN) to join Bloom360 as an independent contractor within a fully virtual, preventive primary care model. You will work with a dedicated panel of patients to deliver personalized, longitudinal nutrition care that is integrated with medical care and focused on improving clinical outcomes.

Your work will support prevention and management of cardiometabolic disease, including obesity, diabetes, hypertension, dyslipidemia, and gastrointestinal conditions, while also promoting overall vitality and longevity. This role emphasizes evidence-based nutrition therapy, behavior change science, and close collaboration with physicians to ensure nutrition is fully embedded into each patient’s care plan.

**What You'll Do **

- Conduct comprehensive virtual nutrition assessments, including dietary intake, metabolic risk factors, social determinants of health, and readiness for change

- Develop and implement individualized, evidence-based nutrition care plans aligned with medical diagnoses, lab values, and physician treatment plans

- Deliver ongoing telehealth counseling using behavior change frameworks (e.g., motivational interviewing, stages of change) to support sustainable lifestyle modification

- Collaborate closely with Bloom360 physicians and care team to co-manage patients with chronic conditions (e.g., diabetes, hypertension, obesity, metabolic syndrome, GI disorders), ensuring nutrition interventions are coordinated with medical care

- Monitor patient progress using clinical outcomes (e.g., A1c, lipids, weight trends, blood pressure) and adjust care plans accordingly

- Provide nutrition support across a range of needs, including weight management, cardiometabolic health, disordered eating recovery, prenatal/postpartum nutrition, and gastrointestinal health

- Maintain accurate, timely, and clinically meaningful documentation in the EHR, aligned with interdisciplinary care

- Participate in care team case reviews, care coordination, and quality improvement initiatives aimed at improving population health outcomes

**What We're Looking For **

**Required Qualifications: **

- Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential — active and in good standing

- Licensed in the state of Michigan (or eligible for licensure)

- Minimum 2 years of clinical or counseling experience post-registration

- Strong comfort with telehealth and virtual care delivery

- Demonstrated experience managing cardiometabolic conditions (e.g., diabetes, cardiovascular disease, obesity, metabolic syndrome) using evidence-based nutrition interventions

- Strong skills in motivational interviewing, behavior change counseling, and patient-centered communication

- Comfort interpreting and applying clinical data (labs, medications, diagnoses) to nutrition care plans

**Preferred Qualifications: **

- Experience in a Direct Primary Care, concierge medicine, or membership-based health model

- Certified Diabetes Care and Education Specialist (CDCES) or similar specialty certification

- Background in intuitive eating, anti-diet approaches, or Health at Every Size (HAES) frameworks

- Cultural competency experience — comfort serving diverse patient populations

- Bilingual (Arabic, Spanish, or other languages) is a strong plus

- Familiarity with HIPAA-compliant telehealth platforms and EHR systems
