Bay Pointe Nursing Home is seeking a compassionate, organized, and dedicated **Social Work Assistant ** to join our team. This role supports residents and their families by helping coordinate care, providing emotional support, and ensuring a positive living environment.

**Key Responsibilities:**

- Assist the Social Services Director with resident assessments and care planning

- Provide support to residents and families during admission, stay, and discharge

- Help coordinate community resources and services

- Maintain accurate and confidential documentation

- Support resident advocacy and ensure psychosocial needs are met

- Participate in interdisciplinary team meetings

**Qualifications:**

- Associate’s or Bachelor’s degree in Social Work, Human Services, or related field preferred

- Previous experience in long-term care, healthcare, or social services is a plus

- Strong communication and interpersonal skills

- Compassionate, patient-centered approach

- Ability to handle sensitive situations with professionalism
