Qualifications
- Bachelor’s or Master’s degree in one of the following fields: Social Work, Psychology, Gerontology, Counseling
- Strong interpersonal and communication skills
- Ability to work collaboratively within an interdisciplinary healthcare team
- Experience working with seniors, disabled adults, or medically complex populations preferred
- Bilingual candidates (Russian, Ukrainian, Spanish, Arabic, Farsi, or other languages) are strongly encouraged to apply
Essential Duties and Responsibilities
- Conduct psychosocial assessments for program participants
- Develop and maintain individualized plans of care in collaboration with the interdisciplinary team
- Provide individual and group counseling interventions in accordance with participant care plans
- Coordinate referrals to community resources and support services
- Serve as a liaison between participants, families, caregivers, and outside agencies
- Participate in interdisciplinary care conferences and team meetings
- Maintain accurate, timely, and compliant documentation and social service records
- Support participant advocacy, emotional well-being, independence, and quality of life
