Essential Functions:
- Evaluate residents’ physical conditions and develop individualized treatment plans
- Provide therapeutic exercises and interventions to improve strength, balance, and mobility
- Collaborate with nursing staff, physicians, and other rehabilitation team members
- Document resident progress and adjust treatment plans as needed
- Educate residents and caregivers on techniques for continued progress
What We’re Looking For:
- Current PA licensure as a Physical Therapist
- Degree from an accredited Physical Therapy program
- Experience in a skilled nursing or long-term care setting preferred
- Strong communication and teamwork skills
- Compassionate and resident-centered approach
