**About the Team**

Michigan Online School (MOS) is a K-12 tuition-free online public school in the state of Michigan. At MOS, we empower students to be their best by enabling them to learn in ways that are right for them – using innovative technology at home, at their proper level, and at their own pace.

**Responsibilities **

- Provides services in speech pathology or audiology in accordance with the physician's plan of treatment. Accepts only clients referred by the Administrator/DON and/or case manager.

- Assists the physician in assessing the client's level of functioning and development (and revision, as necessary) of the plan of care by applying appropriate tests for speech, hearing and language disorders.

- Records and reports the client's reaction to treatment and any changes in the client's condition to the physician, Administrator/DON and/or case manager.

- Instructs and advises the client, family, and other health team personnel in the phases of speech pathology in which they may assist the client.

- Recommends hearing tests by audiologist when necessary.

- Participates in once weekly case conference or in-service

**Qualifications and Skills: **

- Bachelor's degree required.

- Minimum of one year of experience in speech pathology.

- Home health experience preferred.

- Must be license as an SLP in the State of Colorado

- Must have reliable transportation and company-required auto liability insurance.

- Willing to travel within 20 miles radius.

- CPR Certified.

The Facility Licensing and Investigations Section is part of the Healthcare Quality and Safety Branch of the Connecticut Department of Public Health.

PURPOSE OF JOB CLASS (NATURE OF WORK)

In the Department of Public Health this class is accountable for applying professional knowledge as a Dietitian/Nutritionist, Clinical Social Worker, Occupational Therapist, Physical Therapist, or Speech Language Pathologist in the enforcement, inspection, licensure, consultation and implementation or assessment of health care programs and facilities and other licensed entities.

EXAMPLES OF DUTIES

- Provides consultation to state licensed health care facilities and institutions and to unlicensed facilities to bring them into compliance with statutes, work with community institutions and individuals regarding planning, implementation and evaluation of care services, home health services and specialized problems in public health;

- Performs on-site inspection surveys of health care or psychiatric facilities and provides consultation regarding licensure and certification laws, regulations and policies;

- Evaluates quality of services rendered by facility;

- Monitors facilities during strikes;

- Prepares relevant federal and state forms and reports;

- May testify in court relative to findings of inspections and/or investigations;

Four (4) years of experience as a licensed Clinical Social Worker, Occupational Therapist, Physical Therapist or Speech Language Pathologist in a position involving administration, consultation, education or supervision.

- Administrative capacity is interpreted as having managerial responsibility in setting and implementing policies, carrying out program planning, and evaluating and coordinating special projects, etc. in the area specified in the appropriate licensure/certification.

- Consultative capacity is interpreted as the providing of professional expert advice to other than subordinate staff on major programmatic issues in the in the area specified in the appropriate licensure/certification.

- Educational capacity is interpreted as experience in a full time professional educational position instructing professionals in a formalized program or teaching students in an undergraduate or graduate program in the area specified in the appropriate licensure/certification.

- Supervisory capacity is interpreted as having administrative supervision of professional staff.

**Preferred Qualifications**

- Experience organizing data and analyzing care and treatment in accordance with federal and state laws and regulations and professional standards in healthcare.

- Experience with electronic medical records, MS Outlook and MS Word and other electronic databases.

- Experience conducting investigations and documenting facts, findings and outcomes.

- Experience with independently interviewing consumers, health care providers and healthcare beneficiaries.

**Special Requirements**

- Incumbents must possess and maintain one of the following:

- Registration as a Registered Dietitian (RD) through the Commission on Dietetic Registration AND certification as a Dietitian/Nutritionist issued by the Connecticut State Department of Public Health in accordance with Connecticut General Statutes Section 20-206n and 20-206r.

- Licensure as a Clinical Social Worker in the State of Connecticut pursuant to Sec. 20-195 (m) through (r) of the Connecticut General Statutes.

- Licensure as an Occupational Therapist issued by the Connecticut State Department of Public Health in accordance with the applicable Connecticut General Statutes.

- Licensure as a Physical Therapist issued by the Connecticut State Department of Public Health in accordance with the applicable Connecticut General Statutes.

- Licensure as a Speech and Language Pathologist issued by the Connecticut Department of Public Health in accordance with the applicable Connecticut General Statutes.

- Incumbents in this class may be required to travel.
