The Youth and Family Counseling Specialist receives referrals for School and Community Youth Services (SCYS) mental health services for the City of Rockville and provides mental health counseling to local students and their families two to three days per week within elementary schools and potentially middle schools.

- Initiates compliant physician queries when documentation is confusing, ambiguous, or missing and follows up with MD to seek immediate response to query (utilizing the following AHIMA practice briefs as a guide: “Managing an Effective Query Process,” October 2008 and “Guidance for Clinical Documentation Improvement Programs”, May 2010).

As a Social Work Case Manager, there will be times in which you will have to remove children. This can be a difficult and challenging process.

- Assess the ongoing risk to children, develop safety plans, assess the strengths, and needs of the child and family, and make recommendations to various county attorneys and courts.

The Contingency Management mobile team consists of a Program Manager, a Peer Counselor, a Substance Use Counselor and a Nurse Practitioner. The team visits participants at their housing location, develops contingency management goals, assesses progress towards achieving those goals and issues incentives. The team will work closely with the participant’s Housing for Health Intensive Case Management Services Case Manager and property or facility management (as appropriate) to support the participant’s progress towards reducing problematic drug use and to retain housing for participants at-risk of losing their housing due to substance use related behaviors. Abstinence is not required to participate in the program.

- Completes all necessary enrollment procedures and documentation for Contingency Management participants.

- Provides orientation and guidance to Contingency Management participants, and coordinates with staff to ensure collaboration on Contingency Management services.

**Knowledge, Skills & Abilities Required**

- Registered Nurse, Physical Therapist or Occupational Therapist with current license in the state of the assigned territory/cluster (i.e. Illinois, Missouri) required; BSN preferred for Registered Nurses.

- A minimum of five to seven years of appropriate clinical experience with strong clinical assessment skills required; rehabilitation experience or CRRN preferred for Registered Nurses.

- Demonstrated sales aptitude and skills required; proven sales experience with a history of exceeding sales expectations strongly preferred.

- Demonstrated thorough understanding of the SRAlab programs and locations. Ability to communicate a compelling description of these services and how these programs will best meet the patients' needs.

- Demonstrated ability to communicate clearly and knowledgeably with patients, families and clinicians to discuss rehabilitation options.

- Exceptional communication skills, both verbal and written, and polished presentation skills.

- Understanding of excellent customer service and successful proven experience in applying customer service skills.

- Technically competent in using all electronic communications including electronic medical records, lap top computer, iPad, pagers and cell phones.

- Self-directed with ability to successfully and productively work in a home office situation.

- Highly organized, detail oriented, while understanding the 'big picture'; ability to take ownership to successfully meeting strict deadlines.

**Position Overview**

Impact Life is seeking a part-time Case Manager to oversee and provide case management services for residents at a Women's Recovery House located in Wilmington, Delaware. This position is part of our Lower-Level Housing Department and is responsible for supporting residents through individualized case planning, service coordination, and recovery-oriented care.

**This is a part-time 20-hour per week position. The expected schedule is 4 hours per day, Monday through Friday. The specific daily work hours are flexible and will be determined in coordination with the selected candidate based on program needs and availability.**

**Key Responsibilities**

-  Provide direct case management services to assigned residents across two men's housing programs

-  Meet regularly with residents to assess needs, support recovery goals, and monitor overall well-being

-  Develop, implement, and update individualized recovery and care plans in collaboration with the treatment team

-  Coordinate referrals and link residents to appropriate internal and external services, including behavioral health, medical, and social supports

-  Maintain accurate and timely documentation, including required case notes and records

-  Participate in recovery planning meetings and interdisciplinary team coordination

-  Conduct ongoing assessments and adjust services based on resident needs and program protocols

-  Facilitate groups and provide individualized education as required

-  Provide crisis intervention and therapeutic de-escalation when needed

-  Maintain confidentiality and deliver services in a non-judgmental, trauma-informed, and person-centered manner

-  Travel between both Wilmington housing locations as part of the regular work schedule

**Requirements**

**Qualifications**

-  High School Diploma or GED required

-  Bachelor's degree in behavioral health or a related field preferred

-  Minimum of two years of experience in behavioral health, housing services, or a related field

-  Experience providing case management services strongly preferred

**Certification Requirement**

Applicants must either:

- Possess an active Certified Peer Recovery Specialist (CPRS) credential, or

- Meet eligibility requirements to obtain CPRS certification and successfully complete certification within one (1) year of hire
