**Work Schedule: Full-time or Part-time**

**Pay Range: $40.00 - $55.00 / Hour depending on experience and qualifications**

**JOB SUMMARY: **The Speech Language Pathologist reports to the manager/director and performs speech language pathology services (both evaluation and treatment) as assigned by that manager/director.

**JOB DUTIES:**

- Perform patient evaluations and develop treatment plan, including short-term and long-term goals, consistent with physician orders and appropriate for the patient’s overall medical condition.

- Carry out patient care plans or portions thereof, determining appropriate progression of patients within the realm of the established care plan.

- Perform tests and complex treatments individually or with assistance as needed.

- Collaborate with occupational therapists, physical therapists, physicians and other members of the healthcare team relative to individual patient treatment programs and progress.

- Document patient evaluations, treatment progress notes, and discharge summaries for the patient’s medical records in a timely manner as defined by the department policy manual.

- Providing a full range of speech therapy modalities consistent with previous education, experience and clinical needs.

**REQUIREMENTS / EDUCATION / EXPERIENCE:**

- Licensed or eligible for Colorado SLP licensure and Certificate of Clinical Competence from the American Speech-Language Hearing Association

- 1+ years experience in skilled nursing - preferrred

**JOB SUMMARY:**

Under the supervision of a licensed Occupational Therapist, you’ll assist in delivering personalized therapy plans, support daily functional activities, and contribute to a warm, resident-centered environment. **JOB DUTIES:**

- Treat patients as directed by the Occupational Therapist and adhere to facility documentation guidelines

- Record treatments given and progress notes in medical record

- Assist in maintaining department

- Participate in Patient Care and Rehabilitation Conferences

- Assist with cleaning and maintenance of treatment area

- Communicate with supervisor and other health team members regarding patient progress, problems and plans.

- Participate in training programs for other staff in the facility

- Record treatment charges per the facility policy and procedures

- Instruct patient's families or nursing staff in maintenance program

- Any other tasks assigned with or without notice by DOR, Management, or NHA

**REQUIREMENTS / EDUCATION / EXPERIENCE:**

- Associate’s degree from an -accredited Occupational Therapy Assistant program

- Licensed Occupational Therapy Assistant

- This position is open to licensed therapists at all experience levels

- Must be able to fluently read, write, and speak English

- Exhibits a sincere desire to help patients and residents in their rehabilitation process

- Superior interpersonal communication skills

- Strong writing skills required for documentation of the care you provide

- Exhibits knowledge and understanding of therapy practice
