Purpose

To provide nursing care and support to the adult hemodialysis patient with acute kidney injury (AKI) or end stage renal disease (ESRD). Utilize the nursing process to provide nursing care to facilitate the attainment and maintenance of an optimal level of health for patients and families. Organizational Relationships

General supervision is received from Clinical Nursing Director, Clinical Nursing Supervisor and Medical Director.

**Staff Related **- Collaborate and delegate with the Patient Care Technicians (PCT) provision of safe and effective delivery of chronic hemodialysis therapy to patients in compliance with standards outlined in the facility policy procedure manuals, as well as regulations set forth by the corporation, state, and federal agencies.

Participates in staff training and orientation of new staff as assigned.

Assists with medication tracking and inventory as assigned.

Checks availability of adequate emergency equipment and supplies as assigned.

Monitors patient care staff for appropriate techniques and adherence to MM policy and procedures.

**Patient Care ** - Provides safe and effective comprehensive delivery of care to patients with AKI and ESRD.

Completes Initial and Annual Nursing Assessment, and ongoing evaluation and documentation of patient care needs according to Michigan Dialysis Services (MDS) policies and procedures.

Identifies expected outcomes, documents, and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team.

Participate in patient care conferences with the multidisciplinary team and completes all CMS requirements.

Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections.

**Patient Education- ** Ensures educational needs of patients and families are met regarding renal disease.

Provides ongoing education to patients regarding their renal disease, vascular access and dialysis therapy and other related health conditions.

Ensures patient awareness related to transplant and treatment modality options.

Makes referrals to Social Worker and Registered Dietitian as appropriate for individual patient education and counseling.

**Infection Prevention ** - Ensures a clean, safe, and sanitary environment in the dialysis facility patient care area. Ensures competency in the operation of all dialysis related equipment safely and effectively.

Promotes infection control, equipment, and environmental safety.

**Medical Records and Documentation ** - Ensures all relevant data including physician orders, lab results, vital signs and treatment parameters, and patient status are documented appropriately and entered into Medical Information System.

**Professionalism **- Enforces policies and procedures, as well as regulations set forth by state and federal agencies and departments. Delegate appropriate duties to assistive personnel.

**Professional Development/Education for Staff ** - Successful completion of hemodialysis training with satisfactory performance. Children's Emergency Services (CES) is a primary entry point for children 0-20 years of age admitted to C.S. Mott Children's Hospital, a verified level one pediatric trauma center and tertiary facility with referrals from across the state. CES recently expanded to 41 beds to meet the medical/psychosocial needs of our expanding patient population.

CES ED Techs partner with expert pediatric emergency nurses, board certified pediatric emergency medicine faculty, and pediatric residents to provide expert family centered care for over 42,000 children who present each year with injuries and illness across the range of acuity from minor to critical care. The department provides CES Techs with an in-depth orientation, annual mandatory education, and continuing educational opportunities to include maintaining BLS, PALS, and ACLS as well as the opportunity to audit ENPC and TNCC.

**Patient ADLs**

- Assists patients with, or administers routine personal hygiene including baths, oral care, skin care, umbilical cord and circumcision care.

- Performs preventative skin care, pre-operative preps, perineal care.

- Provides toileting/bedpan/urinal; remove/empty foley catheters; weighs diapers.

- Accompanies, escorts, ambulates, moves, turns, dangles, lifts, transports and attends/holds/positions patients.

- Assists with range of motion; applies rehabilitation equipment; releases and applies devices, i.e. wheelchair seatbelt, adaptive equipment, etc., in accordance with specific instructions, to position patients to limit movements.

- Provides patient nourishment by performing such duties as passing and collecting trays, providing snacks/beverages, assists with ordering meals/snacks; feeds patients. Double check breast milk with RN.

**Tube Feedings**

- Perform tube feedings of patients, at the direction of the RN, and after the RN has established placement just prior to each tube feeding (e.g. Change tube feeding bag, prime line and set rate after notifying RN, refill tube feeding bag as indicated per infection control guidelines and after verifying correct tube feeding to be administered).

**Urinary Catheters**

- Performs intermittent sterile urinary catheterizations.

- Performs clean urinary catheterizations (Pediatrics only-waiting for data).

- ?Connects, drains and disconnects urinary catheters.

- Discontinues urinary catheters.

- Assembles intermittent bladder irrigation supplies.

- Applies condom catheter.

- Determines bladder status with Bladder Scanner.

**Enemas/Suppositories**

- Administers cleansing enemas that do not contain medication.

- Applies external Fecal Incontinence Device bags.

**Wound Care**

- Cleanses and redresses non-sterile wounds.

- Performs simple sterile dressing changes, as in central line dressings.

- Assists RN with complex dressing changes.

- Cares for permanent drainage sites and apply appropriate collection device, i.e., ostomy, G- tube,etc.

- Applies binders, ACE wraps, hot and cold applications; provides soaks and special baths.

**Respiratory Care**

- Assist with tracheostomy care on stable tracheotomies including such duties as cleaning and redressing area; assists with tape and tie changes.

- Performs endotracheal/tracheostomy tube suctioning after assessment by RN of patient stability.

- Provides ventilation to the intubated/trached patient with an Ambu bag.

- Assists with Endotracheal tube care and tape changes.

- Assists with P&PD by holding the patient.

Responsibilities*

**Overnight testing procedures ** (also applies to PSGs performed during the day for night shift workers).

- Review existing information in patient record.

- Examine orders and protocols for study.

- Explain test procedures to patients.

- Recognize chief sleep/wake complaint.

- Communicate with ordering physician or Sleep Lab medical staff to clarify unclear or inappropriate orders and communicate information about the patients status.

- Calibrate and adjust equipment to ensure it is in proper working condition.

- Measure and apply appropriate EEG electrodes using the International 10-20 system.

- Apply appropriate EKG (heart), EMG (leg and chin muscles) and EOG (eye movement) monitoring electrodes.

- Apply and securely attach recording transducers, thermocouples, to measure airflow, respiratory effort, apply oximetry probes and CO2 monitors (End Tidal and Transcutaneous).

- Perform physiological calibrations to verify proper functioning of equipment.

- Monitor the PSG and note and correct equipment malfunctions.

- Monitor and change patients body position as needed to obtain the required sleep/wake data.

- End the PSG and wake the patient and carefully remove recording equipment.

- Score the polysomnogram and generate a report summarizing sleep\wake data and physiological events.

- Independently perform specialty studies.

**Patient Equipment and Safety**

- Recognize and respond appropriately to patients medical problems and physiological events, especially cardiac dysrhythmias, respiratory events, and seizures.

- Safely lift and move patient without harm to patient or self.

- Ensure safety and welfare of all patients

- Comply with Universal Infection Control procedures.

- Properly disinfect and sterilize appropriate equipment, electrodes, CPAP (Continuous -Positive Airway Pressure) masks.

- Appropriately handles hazardous materials such as acetone or collodion.

- You will precisely apply electrodes and perform biocalibrations.

- You will work with a variety of patients including adult and pediatric populations.

- You will also set up top-tier equipment and troubleshoot common issues

**RELATED DUTIES**

- Administers oxygen, continuous and BI-level positive airway pressure in accordance with established protocols.

- Understands indications, contraindications, and adverse effects of CPAP, BI-level PAP and oxygen administration.

- Independently performs special procedures including esophageal pressure monitoring, infant/pediatric polysomnography, ambulatory monitoring, and portable (in patient) polysomnography.

- Perform and score PSGs for evaluation of parasomnias and seizures, and prepare report including other clinical events related to parasomnias and seizures.

- Conduct MSLT's and score MSLTs according to established guidelines

- Prepare report of MSLT, including number and time of naps, latency from sleep to onset of REM for each nap, and mean sleep latency.

- Evaluate infant/pediatric patients, and properly perform PSGs for evaluation of sleep apnea, parasomnias and seizures.

- Perform sleep research protocols.

- Perform other duties and lab functions as assigned by supervision

Required Qualifications*

**Polysomnographic Technologist Registered Specific**

- High School graduation with basic coursework or an equivalent combination of education and experience

- Registration as a Polysomnographic Technologist by the Board of Registered Polysomnographic Technologists (BRPT) is necessary. Must maintain registry under BRPT by completing necessary CEUs, as a Registered Polysomnographic Technologist.

- BLS Certification

- This classification requires Primary Source Verification.

**Graduate (Non-Certified) Polysomnographic Technician Specific**

- High school diploma

- BLS Certification.

- Completion of 4 modules of an A-STEP, Accredited Sleep Technologist Education Program.

OR

- Enrollment or completion of a CAAHEP or CoARC-accredited polysomnography education program

Modes of Work

Positions that are eligible for hybrid or mobile/remote work mode are at the discretion of the hiring department. Work agreements are reviewed annually at a minimum and are subject to change at any time, and for any reason, throughout the course of employment. Learn more about the work modes.

Work Schedule

Work scheduled will be assigned by the Sleep Lab Supervisor. Work schedule will include nights shifts during the 24-hour, 7 day/week operations of the Sleep Laboratories. Participation in on call schedule. Travel between Brighton and Ann Arbor locations as required.

Underfill Statement

This position may be underfilled at a lower classification depending on the qualifications of the selected candidate.

Union Affiliation

This position is covered under the collective bargaining agreement between the U-M and the United Michigan Medicine Allied Professionals (UMMAP), which contains and settles all matters with respect to wages, benefits, hours, and other terms and conditions of employment.

Background Screening

Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third party administrator to conduct background screenings. Background screenings are performed in compliance with the Fair Credit Report Act. Pre-employment drug testing applies to all selected candidates, including new or additional faculty and staff appointments, as well as transfers from other U-M campuses.
